Can be applied voluntarily by organisations and support workers in home-care and day programme settings to assist people diagnosed with dementia, their families and representatives. Provides guidance material (Guidance Parts 1 - 4) and sector solution examples in the proposed audit workbook that define what will become current accepted good practice for dementia care in home-care and day programme settings in the community. The focus is on the unique needs of people with dementia, their families and representatives, and the role of support workers in the community. The aim is to promote quality care in the community, and delay or prevent unnecessary moves into residential care. The overarching principles, guidance material and sector solutions specify the dementia community service outcomes that are additional to those required by NZS 8158 Home and community support sector standard and SNZ HB 8158 Home and community support sector standard audit workbook. As this document does not reiterate or duplicate any aspect of the SNZ HB 8158 sector solutions, it will need to be used alongside SNZ HB 8158.
